Criteria:

Southern Domiciliary Care and Rehabilitation Service provides home and community based health and supportive care to people of all ages with moderate to severe disability, and to their carers. The aim is to enhance independence and maintain optimal quality of life. The provision of services which are responsive to consumer needs, innovative and cost effective is paramount.

Criteria:

SDCRS provides Podiatry home visit services to housebound clients in the community. Clients demonstrate complex needs requiring case management.

Cost:

SDCRS fees policy enables financially disadvantaged individuals to apply for a waiver of costs associated with SDCRS services.

Average Waiting Time:

SDCRS wait list is variable. Clients referred are prioritised based on clinical risk status.
